Rhinoplasty – Nose Reshaping Surgery
Many patients seek rhinoplasty at our Utah practice because of dissatisfaction with the shape and size of their noses. Often this dissatisfaction arises from a combination of factors, which can include over-projection, under-projection, a wide or crooked nasal bridge, a nasolabial angle that is too small or large, or a hump in the nose. By altering undesired features of the nose, a remarkable enhancement of patient’s facial appearance can be achieved. Dr. Kennedy’s goal during rhinoplasty is to rebalance and shape the nose so that it is more proportional with surrounding facial structures, as well as to realign and refine the profile for further improvement of facial aesthetics.
The Rhinoplasty Procedure
Rhinoplasty is mostly performed through small incisions on the inside of the nose. However, when greater exposure and control is needed, open rhinoplasty is performed. The open rhinoplasty procedure requires a small incision, which connects to the incisions on the inside of the nose. Through this approach, the cartilage of the nasal tip can be precisely narrowed and resculpted to achieve the patient’s cosmetic goals. Reduction of the bony width of the bridge of the nose can be accomplished through a small intra-nasal incision. Through shaping of the bone, the width can be reduced to match the nasal tip. If the patient is experiencing airway obstruction and requires nasal septum or turbinates surgery to improve breathing, these adjustments can be performed during cosmetic rhinoplasty.
Results of Rhinoplasty
Surgical refinements to the proportions of the nose can lead to dramatic improvements in the overall appearance of the face. Brow Lift – Forehead lift
In many patients, the forehead area and the area around the eyes are the first to show signs of aging. As the natural aging process progresses, the eyebrows can descend to a lower position along the forehead, sometimes settling below the natural brow line. In these cases, the natural arch of the eyebrow is lost and the tail of the eyebrow begins to sag. Horizontal lines across the forehead can begin to form as a result of patients unconsciously raising the eyebrows to relieve heaviness that the sagging eyelids exert on the upper lids. Sagging of the brow can contribute to fullness and heaviness of the upper eyelids as well. Between the brows, vertical frown lines can be an area of concern; build-up of the muscle tissue in this area can create deep creases that can give the face a tired or angry appearance. The goals of brow lift, also called forehead lift, are to elevate the eyebrows, restore the natural arch of the eyebrow, soften horizontal forehead wrinkles, and alleviate the eyebrows’ contribution to heavy, puffy upper eyelids.
Endoscopic Brow Lift
The endoscopic brow lift is designed to elevate the eyebrows and restore the natural arch of the eyebrows by lifting the tail of the eyebrow to a more youthful position. The result is an aesthetically pleasing, youthful arch to the eyebrow. Oftentimes, with the traditional brow lift method, the head of the eyebrow can be lifted to a position above the tail, creating an unnatural, perpetually surprised or overly alert appearance. Typically, elevation of the brows during this procedure softens or helps to eliminate horizontal forehead wrinkles. Endoscopic brow lift generally results in a beautiful outcome with minimal scarring due to the fact that only a few small incisions along the hairline are necessary to complete the lift.
The Endoscopic Brow Lift Procedure
Small incisions are made within the hairline along the scalp through which the endoscope and specially designed surgical instruments are inserted to complete the procedure. The endoscope provides Dr. Kennedy with a view of the structures beneath the skin while he completes the lift of the forehead tissues. During the procedure, the tiny muscles between the eyebrows, which cause fine to deep frown lines to appear, can be surgically removed to relax and smooth the area in an attempt to permanently remove frown lines. These muscles tend to be tenacious, and frown lines -- though reduced -- may return, at which time BOTOX® Cosmetic may be applied to soften or eliminate frown lines. Brow lift is often combined with facelift or eyelid lift to achieve more comprehensive facial rejuvenation.

Eyelid Surgery – Blepharoplasty
Eyelid surgery, also called blepharoplasty, is an extremely delicate procedure that requires superior surgical skill to achieve natural looking results. In this procedure, baggy, puffy eyelids are corrected through the surgical removal of excess skin and fat to rejuvenate the area around the eyes.
The Eyelid Surgery Procedure
During the procedure, excess eyelid skin is removed in such a way that the final scar is well hidden along the natural line of the skin and within the crease of the upper eyelid. Rejuvenation of the lower eyelid is performed in a similar manner, with excess skin and muscle tissue being removed in addition to puffy fat deposits. The incision for the lower lids is strategically placed along the lower lash line. In certain younger patients who only have issues involving excess fat, lower lid rejuvenation can actually be accomplished through an incision made on the inside of the lower lid. In the lower eyelid, there are limitations on how much skin can safely be removed.

Cheek Augmentation – Malarplasty
Strong, prominent cheekbones are an essential component of a handsome or beautiful face. If there is a facial characteristic that is thought to be universally beautiful, then it must be high, well-defined cheekbones. When cheekbones are sallow or when the mid-face (the region just below the lower eyelids) appears flat, the face can appear tired and reveal signs of aging sooner. A weak, flat facial bone below the eye emphasizes aging lower lids, making their baggy appearance more prominent. By placing a solid silastic cheek implant through a small intra-oral incision (a technique that does not create any external scars on the face) the cheeks become lifted and more defined, exhibiting enhanced volume. The cheek implant sits directly on top of the facial bone and creates very natural elevation of the overlying tissue. The results of cheek augmentation look completely natural and symmetrical once healing is complete. Neck Lift and Liposuction of the Neck
Excess skin and unwanted fat deposits under the chin and along the neck can give the appearance of fullness or sagginess. Aside from overweight patients or those with age-related droopiness along the neck and chin, many younger patients with normal or ideal body fat have excess fat in these areas due to heredity or other factors. For these patients, liposuction of the chin and neck slims and tightens the area to improve its appearance. When performed properly, liposuction creates smooth contours with minimal unevenness or dimpling. Generally, chin and neck liposuction is performed through a single three-millimeter incision in the shadowed area below the chin. The entire neck and jowl area can be treated through this single small incision, resulting in a tiny, inconspicuous scar. Liposuction of the neck is often performed simultaneously with neck lift, during which Dr. Kennedy tightens the muscles of the neck and removes the excess skin that causes sagginess. To complete neck lift with smooth, natural results, facial liposuction techniques provide an ideal method for refining and finishing the initial outcome of the lift. Lip Enhancement
Full, supple lips are associated with youthful, glamorous faces. Both young and aging patients come to our practice desiring fuller lips. Young patients may be unhappy with the natural thinness of their lips, whereas older patients often seek to reverse age-related thinning of the lips by undergoing lip enhancement to restore their youthful pout. As lips age and flatten, they lose their pout and fullness along the border (called the vermillion border). Lip enhancement procedures are designed to increase lip fullness and volume in addition to enhancing lip definition. There are several techniques used to augment the size and shape of the lips, ranging from injectable dermal fillers and fat transfers to surgically implanted materials such as Gore-tex®:
Injectables
Restylane®, an FDA approved hyaluronic acid filler, collagen can be injected along the lip border to enhance the fullness of the lip and alleviate flatness along the vermillion border. Although some patients have reported the effects of their treatment to last for up to a year, Restylane® generally lasts for approximately four to six months before a touch-up is needed to maintain results. Likewise, collagen enhancements last approximately six months before repeat treatment is needed.
Fat Transfer
Fat is “harvested” (removed) from an area of the body, such as the abdomen and thighs, and then processed and re-injected into the lips to create fullness. Using this technique, a dramatic increase in lip volume can be achieved. With fat re-injection it is hoped that some of the transferred fat cells survive in their new location and provide a long lasting enhancement. Survival of the fat cells is not 100 percent, often making it necessary to perform a second or third transfer before an ideal amount of live fat cells have accumulated in the area. When fat transfer is successful, it provides long lasting results
that feel naturally soft and supple.
Implantables
Gore-tex® is a surgically implantable substance that has been used for more than twenty years in vascular surgery. The substance is well tolerated by the body and will not be reabsorbed or dissolved over time – it is permanent. To complete the procedure, strands of Gore-tex® are threaded through inconspicuous incisions in the corner of the mouth. Multiple strands can be implanted at one time to gain the appropriate degree of augmentation. The Gore-tex® strands are not rigid, but they do have enough substance to lessen the effects of habitual lip pursing to reduce further wrinkling around the mouth. This procedure can be paired with dermabrasion around the outside of the mouth to further diminish lip-distorting wrinkles.
